What to Expect During Your First Visit
Your initial appointment will consist of a full examination, updating x-rays, and explanation of treatment options.
Please Assist Us by Providing the Following Information at the Time of Your Consultation:
- Bring a list of your current medical prescriptions.
- If you have medical or dental insurance, bring your identification cards.
- When you call our office to schedule your initial appointment you will be asked for your insurance information, including the name of the insurance subscriber, insurance company, employer, and identification numbers (insurance ID or social security number). As a service to our patients, we will attempt to verify your insurance eligibility and benefits prior to your appointment.
- Bring completed "New Patient" paperwork or arrive 15 minutes early to complete in office
- "Records Release" for x-rays needs to be submitted prior to 1st appointment if having x-rays sent digitally from a previous office.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
